Coaches Record Form (CRF) 

At every NCCP Course each LF must have participants complete the CRF fully.

 An Incomplete form is defined as:

• There is no participant category selected
• There is either old or new selected
• There is no name for the workshop or activity filled in 

Any of the following details are omitted:

• Organization
• Date of workshop
• Workshop location (city and province/territory)
• Date of form approval
• Name or sport/government representative
• There is no person identified as the Learning Facilitator or Evaluator

Any of the following Learning Facilitator/Evaluator details are omitted:

• C#
• Name
• Address
• City
• Province / Territory

Any of the following coach details are omitted for any coach on the form*:

• Surname
• First Name
• Street address
• City
• Postal Code
• Date of Birth

Fully completed CRF's from courses are to be sent to Fran Mason. Fran will check all details and submit to Rugby Canada and Coaches Association of Canada. 

LF Expenses 

Expense Form

LF's are an integral part of the Education Workforce they deliver NCCP Courses and are compensated well for their education skills, rugby knowledge and ability to deliver practical coaching courses.

LF's are to complete as soon as possible after course delivery and send to Technical Director - ahall@rugbyontario.com 

Honorariums 

Mileage 

Rugby Ontario will pay for travel to courses. 

Mileage Model

Accommodation 

Rugby Ontario will pay for accommodation on certain occasions this will be determined by the Technical Director on a case by case basis.
